About This Service

Our jaw pain physiotherapy service treats the problems clinicians group under temporomandibular disorders, or TMD. That umbrella covers muscular pain in the chewing muscles, soreness arising from the joint itself, a disc that slips with or without recovering its position, and wear-related change in the joint surfaces. Working out which pattern you have is the first job of the assessment, because the treatment differs. Clenching and night-time grinding, stretches of high stress, hours held in one head-and-neck position, and an irritable upper neck all feed jaw symptoms, so a sensible plan tackles several of these at once rather than the joint in isolation. If your jaw clicks but is painless and opens normally, that usually needs no treatment, and we say so.

Treatment is hands-on and active in roughly equal measure. Manual therapy releases the masseter, temporalis and pterygoid muscles and restores glide in the joint; intra-oral techniques are performed only after we have explained them and you have given explicit informed consent, and turning them down does not limit what else we can offer. Alongside that you learn mobility and motor-control drills for the jaw — controlled opening, tongue resting position, quiet nasal breathing — because rebuilding steady, symmetrical movement is what holds the gains. The neck is treated as part of jaw care, since the top cervical segments refer pain straight into the jaw, temple and ear. We also work through parafunctional habits: gum, nails, pens, phone-cradling and daytime clenching you may not even notice.

Most TMD settles with conservative care, and physiotherapy is a reasonable first step before splints, injections or surgery come into the conversation. Where a night guard would help control grinding, we coordinate with your dentist — that collaboration is routine, not an exception. In Ontario you can book without a doctor’s referral, although a few insurance plans still ask for one. When Should You Get This Treatment?

  • Your jaw aches when you chew, yawn or talk for long. That is the commonest reason people book jaw physiotherapy.
  • Your mouth no longer opens as wide as it did. Tight muscles and a restricted joint glide both need testing.
  • The clicking has started to hurt. Painful joint noise deserves assessment; a silent, comfortable click does not.
  • You wake with headaches, tight temples or tender teeth. Night grinding is often behind this, and dentists help manage it.
  • Your ear feels blocked or sore but is otherwise healthy. The joint and deep chewing muscles refer pain there.
  • Jaw pain arrived alongside a stiff, aching neck. Neck and jaw symptoms travel together, so we assess both.
  • Chewing tires your cheeks before the meal is finished. Muscle fatigue, not joint damage, usually explains that.
  • Your jaw stiffened after dental work or wisdom-tooth removal. Lingering restriction responds to graded mobility work.
  • Jaw symptoms began after a car collision. This is a recognised part of whiplash and your auto insurer is billed.
  • Your dentist fitted a night guard and wants the muscles treated. Splint and physiotherapy work well together.
  • Injections or surgery have been raised and you want another option. Reversible TMD treatment deserves a proper trial first.
  • See a dentist or doctor first, not us, if your jaw is locked shut, pain followed a blow to the face, there is unexplained swelling, or your teeth suddenly meet differently.

What to Expect

Your first jaw appointment lasts about an hour, in a private room. We start with the story: when the jaw first bothered you, which side, whether the pain is constant or comes in waves, and what reliably sets it off. We ask about grinding, clenching, sleep, stress and workload, headaches, ear symptoms, and any dental history that could matter — braces, extractions, implants, long procedures, jaw surgery. We also ask what you want back most, whether that is eating a normal meal or sleeping without waking sore.

Next comes the physical examination. We measure how far your mouth opens with a ruler between the front teeth and watch whether the jaw tracks straight or swings sideways. We check side-to-side and forward movement, feel and listen for joint noise through the full range, and load the joint gently to separate joint pain from muscle pain. We then palpate the masseter, temporalis and pterygoids, and screen your upper neck, shoulders and head position, since a stiff neck commonly travels with a sore jaw.

Then treatment begins, usually on day one:

Hands-on work outside the mouth. Sustained pressure and slow release through the masseter, temporalis and the muscles around the cheekbone and temple, plus soft-tissue work down the side of the neck.

Release from inside the mouth, only if you agree. Some chewing muscles, notably the pterygoids, can only be reached intra-orally with a gloved finger. We explain exactly what is involved, obtain your explicit informed consent beforehand, and stop the moment you ask. Declining is entirely fine — the rest of the plan still works.

Joint mobility. Gentle distraction and guided gliding of the jaw joint to restore capsular movement and smooth out the path your jaw takes when opening.

Neck treatment. Mobilisation of the upper cervical joints and release of the muscles at the base of the skull, because those segments refer pain into the jaw, temple and ear.

Jaw control and home exercise. You practise opening slowly and evenly with the tongue resting on the palate, breathing through the nose, and holding the jaw in a relaxed position. Two or three short drills go home with you, with video if that helps.

Habit change, plus optional add-ons. We map where the clenching happens in your day and build small interruptions into it. Acupuncture is available as an extra for stubborn muscular pain from Dr. Thessa Prashad, DC, who is certified in McMaster Contemporary Medical Acupuncture.

How long it takes. Many muscular jaw presentations ease noticeably within the first handful of visits, and a typical course runs six to twelve weeks. A jaw that locks takes longer and is managed alongside your dentist or an oral surgeon.
