How can I tell if my wrist is broken or just sprained?
You often can't tell reliably at home, which is why certain findings should send you for an X-ray rather than a wait-and-see approach. Both injuries usually start the same way — a fall onto an outstretched hand, which clinicians shorten to FOOSH — and both swell and hurt. A sprain stretches or partially tears ligament, so pain is spread across the joint, movement is uncomfortable but possible, and grip is weakened rather than gone. A fracture involves bone, so tenderness is typically pinpoint — pressing directly on one spot reproduces a sharp, unmistakable pain. Marked swelling within the first hour, any visible bend or step in the contour of the wrist, or an inability to hold even a cup all raise the odds of a break. The distal radius — the wider forearm bone on the thumb side — is the bone adults fracture most often in a FOOSH. If any of those signs are present, get an X-ray first; physiotherapy comes after bone injury has been ruled out or properly managed. Why is the scaphoid bone so dangerous to miss?
Because a broken scaphoid can pass as a mild sprain — and an untreated one can quietly destroy the joint. The scaphoid is a peanut-sized carpal bone at the base of the thumb that takes much of the load when you land on your palm. Two things make it treacherous: the pain is often modest, so people keep using the hand for weeks assuming it's a sprain, and a fresh scaphoid fracture frequently does not appear on the initial X-ray — the crack can stay invisible for a week or two. That's why a "clear" X-ray after a fall doesn't close the case if pain persists: standard practice is to protect the wrist in a cast or splint and repeat imaging before declaring the bone healthy. The stakes are real — the scaphoid has a fragile blood supply, and a missed fracture can fail to unite or lose circulation entirely, leading to collapse of the bone and early wrist arthritis. The tell-tale finding is tenderness in the anatomical snuffbox, the small triangular dip visible on the thumb side when you extend your thumb. What is a TFCC tear?
A TFCC tear is damage to the triangular fibrocartilage complex, a hammock of cartilage and ligament on the pinky side of the wrist that cushions the joint and stabilizes the forearm bones as they rotate — the wrist's version of the knee's meniscus. Tears happen suddenly, from a fall or forceful twist, or gradually, from years of loaded rotation in trades work, racquet sports, or gym training. The pattern is distinctive: pain on the ulnar side, the edge below the little finger, and pain with forearm rotation — turning a key, opening a stubborn jar, using a screwdriver — often with a click or soft clunk deep in the wrist. The most telling complaint is pain when bearing weight through the flat hand: pushing up from a chair, a push-up, or a yoga pose loads the TFCC directly and reproduces the symptom almost every time. Because plain X-rays show bone rather than cartilage, TFCC injuries are routinely mislabelled as lingering sprains for months. Most tears respond well to conservative care — a period of protection from loaded rotation, then progressive strengthening of the muscles that stabilize the forearm — with surgery reserved for the minority that stay unstable or painful despite proper rehabilitation.
Why does my thumb side of the wrist hurt?
If there was no fall involved, the usual culprit is De Quervain's tenosynovitis — irritation of the two tendons that pull the thumb outward, where they pass through a snug tunnel at the thumb-side edge of the wrist. When those tendons work harder than they're used to, the tunnel's lining thickens and every thumb movement starts to sting. The classic sufferers are new parents — hence the nickname "mommy thumb" — because lifting a baby under the armpits, thumbs spread wide, dozens of times a day is precisely the movement that overloads these tendons. We also see it in people who've ramped up phone use, gardening, hairdressing, or assembly work. The screening test is simple: tuck your thumb inside your fist and tilt the wrist toward the little finger. Sharp pain along the thumb side is a positive Finkelstein test and strongly suggests De Quervain's. The key distinction is between this tendon pain and scaphoid tenderness — tendon pain traces along a line and flares with thumb movement, while scaphoid tenderness sits deep in the snuffbox after a fall. Could my symptoms be carpal tunnel syndrome instead?
If the dominant symptom is tingling or numbness rather than pain, think nerve rather than tendon or ligament. Carpal tunnel syndrome is compression of the median nerve on the palm side of the wrist, and it announces itself in a recognizable way: pins and needles in the thumb, index, and middle fingers — typically sparing the little finger — that is often worst at night. Many people describe waking to shake the hand out, or dropping small objects as grip becomes unreliable. Symptoms flare when the wrist stays bent for long stretches: driving, holding a phone, sleeping with the wrist curled. The distinction matters because the treatment differs — night splinting to keep the wrist neutral, nerve-gliding exercises, and fixing the workstation habits that keep the wrist flexed. Most mild and moderate cases improve without surgery when treatment starts early.
Can typing or manual work cause wrist tendinopathy?
Yes — wrist tendons can become painful when their workload rises faster than they can adapt, with no single injury moment at all. Office workers at a poorly arranged keyboard and mouse, warehouse staff doing repetitive lifting and scanning, and tradespeople gripping tools for hours are the typical stories we hear. The pain builds gradually, feels worse at the start of activity or the morning after a heavy day, and eases once the tendon warms up — quite different from the constant ache of a fresh sprain. The counterintuitive part of treatment is that pure rest rarely fixes a tendinopathy: unloaded tendons don't strengthen, they just hurt again when work resumes. Treatment follows the diagnosis, but nearly every wrist program moves through three phases at a pace set by the tissue involved. First, protection: calming pain and swelling, and shielding the healing structure with a splint or brace where one helps — a thumb spica for De Quervain's, a support limiting loaded rotation for a TFCC tear, a night splint for carpal tunnel. When the wrist can meet the specific demands you're returning to — not simply when it stops hurting at rest, which happens much earlier. For manual jobs, it means demonstrating the grip strength, lifting capacity, and tool tolerance the role actually requires — tested directly, not assumed. Not necessarily. Early X-rays can miss a scaphoid fracture, and they don't show ligament or TFCC damage at all. Ongoing pain weeks after a fall — especially tenderness at the base of the thumb — deserves re-examination and possibly repeat imaging, arranged through your physician.
Do I need a doctor's referral to have my wrist assessed?

Is clicking in my wrist something to worry about?
